Located at 754 E 151st St, this health center provides integrated services for the whole family, and features new and expanded pediatric and women’s health wings.

Casa Maria Health Center

Located at 324 E 149th St, Casa Maria Community Health Center provides integrated primary care, behavioral health, and additional services to individuals of all ages.

Claremont Health Center

Located at 262-4 E 174th St, Claremont Family Health Center provides integrated primary care, behavioral health, and additional services to individuals of all ages.

Park Avenue Health Center

Located at 4196 Park Avenue, Park Avenue Health Center provides integrated primary care, behavioral health, and additional services to individuals of all ages.

Clay Avenue Health Center
